To submit a formal complaint, download a copy of the "Patient Complaint Form" linked below. Complete the top portion of the form and save it. Navigate to the link for the Google Form titled "Patient Complaint/Grievance." Submit the Google Form with your name, phone number, and attach the form. A member of the PEP administration team will contact you as soon as possible. You may also call the PEP office to submit your complaint through the phone and have a PEP staff member fill out the form on your behalf.
